Ontario accident benefits
OCF-19 applications, completed by our physician
The OCF-19 is the Application for Determination of Catastrophic Impairment under Ontario's accident benefits. A physician must complete the medical portion. Our physician completes it after a thorough review of the patient's records and medical history.
How we complete the OCF-19
- Full record reviewAll clinical notes, hospital records, imaging and specialist opinions.
- Complete medical historyA virtual consultation covering the injury, its course and current function.
- Additional questionnairesWhere clinically needed.
- The OCF-19, completed and signedBy our physician.
What we do and don't do
We complete the OCF-19 application as the patient's treating practice. We do not run multidisciplinary catastrophic impairment assessments. If those are needed, they're conducted separately by specialist assessors.
What to send
As complete a record as you have: hospital and rehab records, imaging, specialist opinions and prior treating notes.
