Frequently asked questions
What does Circle Bay Medical do?
Circle Bay Medical is an independent virtual medical practice for people injured in accidents or living with a disability. Our physician-led medical team provides consultations, clinical records, MIG reports, OCF-19 completion, OCF-18 treatment plans, physician-completed forms, specialist referrals and imaging.
Who do you help?
People who can't get the medical care or forms they need: patients without a primary care physician, and patients whose physician can't take on accident-related care or disability forms. Most referrals come from law firms, case managers and rehabilitation providers.
How quickly can a patient be seen?
A first virtual visit is typically within one week of referral.
What forms do you complete?
CPP-D, the Disability Tax Credit, ODSP, LTD and STD statements, OCF-3, OCF-18, OCF-19, Trillium, Atlantic accident benefits forms, WSIB and WCB forms, Veterans Affairs forms and Excessive Demand medical opinions. If a form needs a physician, we can complete it.
What is a MIG report?
A MIG report is a physician's clinical opinion on whether a patient's injuries fit within Ontario's Minor Injury Guideline, based on a full record review and a virtual consultation.
Can your physician complete an OCF-19?
Yes. Our physician completes the OCF-19 Application for Determination of Catastrophic Impairment after a full record review and medical history. We do not run multidisciplinary catastrophic impairment assessments.
Are you registered with HCAI?
Yes. We submit OCF-18 treatment plans through HCAI for people injured in Ontario car accidents.
Is there a cost to the patient?
In Ontario, care for people injured in a car accident is usually funded through their auto insurer via HCAI, and we confirm funding before care begins. Outside Ontario, we confirm how care will be funded before the first visit.
Are you independent?
Yes. We are the patient's treating practice, not a retained assessor. Our duty of care runs to the patient.
Who provides the care?
A physician-led medical team, licensed in the provinces we serve.
How do I refer a patient?
Use our online referral form, email referrals@circlebaymedical.com, or send an OCF-1 or police report. You can upload records securely.
Can a patient contact you directly?
Yes. Call 1-800-830-6629 or email info@circlebaymedical.com. We'll work with the patient's legal, insurance or rehabilitation team to arrange care.
Where do you provide care?
Ontario, Nova Scotia, New Brunswick, Newfoundland and Labrador, Prince Edward Island, British Columbia and Alberta, all by secure video.
What specialists can you refer to?
Psychiatry, physiatry, orthopedics, physiotherapy and occupational therapy, along with imaging such as MRI and CT when clinically appropriate.
