Privacy Policy

Circle Bay Medical Corp operates a virtual medical practice. We are a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA), and we handle personal health information under the applicable health privacy laws of each province where we provide care, including Nova Scotia, New Brunswick, Newfoundland and Labrador, Prince Edward Island, British Columbia and Alberta, and the federal Personal Information Protection and Electronic Documents Act (PIPEDA) where it applies.

What we collect

Personal health information, including identifying information (name, date of birth, health card number where applicable), contact information, clinical history, records received from other health care providers and organizations, and information generated during consultations.

Why we collect it

To provide medical care, to prepare clinical records and medical documentation, to coordinate specialist referrals and imaging, and to meet our legal and regulatory obligations as health professionals.

Who we share it with

With the patient's consent, we share clinical records with the parties authorized to receive them, typically the referring law firm, case manager or health organization. We share information with specialists and other health care providers when referrals are made, and with insurers where the patient has authorized it or the law requires it. We do not sell personal health information, and we do not share it with advertisers.

How we protect it

Clinical information is stored on secure systems appropriate for health information. Access is limited to authorized personnel. We maintain administrative, technical and physical safeguards consistent with applicable privacy law.

Where information is stored

Our online referral and secure upload forms are provided by JotForm, and information submitted through them is stored on JotForm's secure servers in the United States. Clinical records are kept in our electronic medical record system. We use service providers that maintain safeguards appropriate for health information.

Your rights

You have the right to access your records, request corrections, withdraw consent for specific uses, and file a complaint with the Information and Privacy Commissioner of Ontario or the privacy commissioner in your province.
